Introducing Track to Change

Track to change

MEL for complexity, not compliance

Track to Change is Clear Horizon’s digital Measurement, Evaluation and Learning (MEL) platform designed to help teams collect, organise and learn from real-time signals of change in complex initiatives.

It enables changemakers to harness the rich, often-missing qualitative data that brings impact stories to life.

Who we are

A values-led company striving for social justice and environmental sustainability.

We show up with care and integrity to help communities and organisations understand what’s changing and why it matters.

Our team has worked on hundreds of initiatives to strengthen positive impact for people, places and planet. This depth of collective experience is something you can lean on when working with us wherever you’re based.

About us

Our approach

About us

Collaboration and participation

We work alongside people, not over them, grounded in respect, learning and genuine partnership. We listen, stay curious, and co-design the work so communities and organisations always have ownership of their change endeavour. Our role is to enable, not steer.

Clarity in complexity

We bring practical know-how to complex work, helping teams see what’s shifting, and how to learn from those shifts. We cut through noise, make sense of emerging change, and offer tools that are actually usable in messy, real-world settings.

People first

We make MEL human, blending stories, and systems thinking to support meaningful, lasting change. We value lived experience and weave it together with data to help people understand impact in a way that’s authentic and genuinely useful.

A slow-cooked approach to evaluating community-led change

Testimonials

Just want to thank you so much for both the push and the support re entry in the SIMNA awards. So humbling and amazing that we have been shortlisted with 5 others - and are they big, long term organisations! Wow. We won't win against that level of maturity and investment, but delighted to have received such a high level of attention and recognition as a finalist. So much of our practice has grown out of Clear Horizon training, coaching and modelling. As an ex teacher we had a saying in the field that 'modelling is not only a way of teaching- it is the best way of teaching' and your organisation does this through your evaluation work, your online academy and how you uphold your principles. So thanks again for all of the excellence that you generate as you go about your work.
— Fiona Merlin, Measurement, Evaluation, and Learning Coordinator, Hands Up Mallee
Over the past three years, the Clear Horizon team has played an integral role in evaluating Multicultural Communities Council of SA’s community-led initiatives, including the Multicultural Men’s Mental Health Community Connections Discovery Project and the Taking Care of Me Health Literacy Project. The Adelaide-based team has consistently demonstrated cultural responsiveness by taking the time to listen, learn and adapt evaluation processes that respect the lived experiences and voices of CALD communities. Working alongside community connectors and Health Champions, they have co-designed measurement frameworks and participatory evaluation tools that genuinely reflect community realities. Clear Horizon’s approach, grounded in collaboration, humility and continuous learning has strengthened our ability to measure impact in ways that are culturally safe, meaningful and aligned with our vision for community-led health and wellbeing.
— Ukash Ahmed, Multicultural Communities Council of South Australia
